Yes — in fact, long-term patients are exactly who this program is designed for. Medications taken for many years require especially careful, slow tapering. Standard protocols are often far too fast for people with long medication histories. Dr. Vania's approach is built around your individual history.

This is a cash-pay, out-of-network practice. Upon request, Dr. Vania provides a superbill you can submit to your insurance for potential partial reimbursement. HSA and FSA funds are also typically applicable.

Dr. Vania works with the full range of psychiatric medications — antidepressants, benzodiazepines, mood stabilizers, antipsychotics, stimulants, and many commonly prescribed off-label medications. Eligibility is assessed during your initial evaluation.

Dr. Vania Manipod, D.O.

Dr. Vania Manipod is a board-certified psychiatrist and osteopathic physician with over 14 years of clinical experience, with a clinical focus on medication optimization and deprescribing that began during her psychiatry residency.

She founded this practice to give patients something that is genuinely rare: a physician whose entire clinical focus is helping people reduce and discontinue psychiatric medications safely, thoughtfully, and at a pace that actually works for their lives.

Why she built this practice

During her psychiatry residency, Dr. Vania observed cases where deprescribing — rather than prescribing more — was a critical component of a patient's treatment plan. Recognizing the need long before it became a mainstream conversation, she co-authored a peer-reviewed research article on the topic during her training: "Treating Anxiety by Discontinuing Antidepressants: A Case Series" — evidence that this work has been central to her clinical thinking from the very beginning of her career.

In 2015, she wrote publicly about the severe withdrawal effects patients experienced when medications were abruptly discontinued, and was subsequently added to a patient-created list titled "doctors who are open to discontinuing antidepressants." It was then that she realized the scale of the unmet need. Over the years, patients have regularly sought her out specifically for help tapering. OffScript Psychiatry is the formalization of that work — a dedicated space for patients who deserve a physician who will actually help them try.

Most tapering protocols fail because they go too fast. Standard guidelines often recommend reducing by 10–25% every few weeks — tolerable for some, harmful for others. Dr. Vania's approach is different: reductions are smaller, timelines are longer, and the plan adjusts based on how your body is actually responding.

Initial Evaluation — $650

A 60–75 minute comprehensive virtual appointment. By the end, you will have a clear, personalized tapering roadmap. Includes any initial prescriptions needed.

Your Initial Evaluation is a standalone appointment. If you'd like to take time before beginning the Tapering Program, you may select your program start date within 60 days of your evaluation — your monthly billing begins only when you're ready.

Tapering Program — $450/month

The core program. Includes a monthly 30-minute virtual appointment and weekly 10-minute check-in calls. Dr. Vania manages all prescriptions and adjustments. A 6-month minimum is required — not as a contract, but because safe tapering requires genuine time to build an accurate clinical picture and adjust your protocol meaningfully.

Maintenance — $300/month

For patients who have completed their active taper. Monthly sessions support the post-taper transition. Recommended at Dr. Vania's clinical discretion — not sold as an add-on.

A note on safety

This involves real medical care, including prescription management. Dr. Vania will not begin a taper she doesn't believe is clinically appropriate. Your stability comes before any other goal.

About the program

Who is this for? Adults in California who want to safely reduce or discontinue psychiatric medications under careful medical supervision, at a pace their nervous system can tolerate.

What medications can Dr. Vania help with? Antidepressants, benzodiazepines, mood stabilizers, antipsychotics, stimulants, and many off-label psychiatric medications including gabapentin, hydroxyzine, and clonidine. Eligibility assessed at your initial evaluation.

How long does tapering take? Anywhere from 6 months to several years. It depends on the medications, how long you've been on them, and how your body responds. There is no predetermined timeline.

Can I taper multiple medications at once? Generally no. Dr. Vania typically recommends tapering one medication at a time to accurately identify what each change is producing. Exceptions are assessed individually.

Do I have to start the Tapering Program right after my Initial Evaluation? No — life happens. After your Initial Evaluation ($650), you have up to 60 days to select your Tapering Program start date. Why a 6-month minimum? Because safe tapering takes real time. Six months is the realistic floor for most tapers, and this period gives Dr. Vania time to build an accurate clinical picture of your response and adjust your protocol meaningfully. After six months, the program continues month-to-month.

Safety

What if I need to slow down or stop? Always an option. Your taper plan is a living document. If you're struggling, Dr. Vania will adjust, pause, or in rare cases recommend reinstatement. Your stability is the goal, not a finish line.

Will Dr. Vania coordinate with my other providers? With your permission, yes. She is happy to communicate with your therapist, PCP, or other treating clinicians when it supports your care.

Cancellation

What if it turns out not to be the right fit? After completing the initial 3-month minimum, you may cancel with 30 days written notice. Dr. Vania will work with you to ensure a safe transition of care — including documentation, referrals, and any prescriptions needed to maintain stability. No one will ever be left without a plan.

What happens to my prescriptions if I cancel? Dr. Vania will ensure you have sufficient medication and documentation to transition safely to another provider. She will never abruptly discontinue care in a way that could jeopardize your stability.

Once a physician-patient relationship is established, all protected health information (PHI) and electronic protected health information (ePHI) is handled in accordance with the Health Insurance Portability and Accountability Act of 1996, as amended (HIPAA). We are committed to protecting your medical information and will use it to the minimum necessary to accomplish the intended purpose of any use, disclosure, or request.

Clinical communications occur exclusively through HIPAA-compliant platforms. A full Notice of Privacy Practices (NPP) will be provided to you at the time of your initial evaluation, which you will be asked to acknowledge in writing.

Uses and Disclosures

Your protected health information may be used for treatment, payment, and healthcare operations without additional written authorization. This includes appointment reminders, coordination of care with other treating clinicians (with your permission), and billing-related communications. Your medical information will not be used for marketing purposes or sold to any third party without your explicit written authorization.

Certain disclosures may be made without your authorization as required by law — including reporting of communicable diseases, instances of suspected abuse or neglect, or situations involving immediate threat of danger to you or another person.

Your Rights

You have the right to inspect, copy, and request amendment of your medical records; request restrictions on certain uses and disclosures; receive an accounting of disclosures; and receive notification in the event of a breach of your unsecured protected health information. All requests must be made in writing to the Privacy Officer.

Breach Notification

In the event of a breach of your unsecured protected health information, you will be notified in writing without unreasonable delay and no later than 60 days following discovery of the breach, in accordance with the HITECH Act.

Third-Party Services

This website is hosted by Netlify. Patient scheduling, clinical documentation, and secure messaging are managed through HIPAA-compliant third-party platforms that have signed Business Associate Agreements (BAAs) as required under HIPAA.
